CDC setup and sequence

Use a stopwatch, keep eyes open, and do not use a cane or walker. A helper should stand close enough to assist. Time each position until 10 seconds, the feet move, or support is needed.

  1. Feet side by side.
  2. Semi-tandem: instep of one foot touching the other foot’s big toe.
  3. Tandem: heel touching the other foot’s toes.
  4. Stand on one foot.

Proceed only after a 10-second hold and stop after the first unsuccessful stage. This protocol does not define a 0–4 clinical score, so none is reported here.

For a separate self-assessment, complete the CDC Stay Independent Fall Risk Questionnaire.

Safety: Have another person close enough to assist. Stop if the person becomes unsteady, needs support, or has concerning symptoms. This educational recording tool is not a diagnosis and does not predict whether a particular person will fall.
